*Exclusions apply: This plan is only valid at Tooth Clues, PLLC. Enrollment into the plan is per
individual and cannot be combined with any private insurance coverage. If the patient fails to
utilize any of the included services during the annual coverage period, there is no refund of
premium and any unused services will be forfeited as there is no carry-over period. Prices and fees
are subject to change.
